Through our unique, patent pending, and secure verification system, hospitals, courts and authorized individuals can access critical documents—without hassle, subscriptions, or outdated filing systems.
Here’s how it works:
Please prepare the following information/documents for your registration:
1) You will be asked to fill out your personal information, including your name, date of birth, ID number, phone number, email address, and home address.
2) You will also be asked to fill out information on the individuals you select (“designated individuals”) who will have access to some or all of your documents. You may select zero to five designated individuals. Information requested will include name, mobile number, email, optional landline number, optional home address, and optional relationship.
3) You may upload up to 12 documents. PDF, JPG, PNG, and other image formats will be accepted. Suggested documents include List of Emergency Contacts, List of Allergies and Medications, Advance Healthcare Directive, Will, Trust, and Power of Attorney.
With the above information/documents gathered, you are now ready to register.
Just sign up with basic personal information. We’ll never share your data without your explicit permission.
Add up to five individuals—family members, friends, or advisors—who should be able to view some or all of your documents.
Upload advance directives, power of attorney, will or any other documents that you want on hand in an emergency or legal situation.
For each document, choose exactly which designated contacts can view it.
