At RegistryAZ™, we make it simple and secure for you to store your critical health-related and legal documents—and to share them only with the people and institutions you choose. Through our unique, patent pending, and secure verification system, hospitals, courts and authorized individuals can access critical documents—without hassle, subscriptions, or outdated filing systems.

Here’s how it works:

Create Your Profile

Just sign up with basic personal information. We’ll never share your data without your explicit permission.

Designate Individuals

Add up to five individuals—family members, friends, or advisors—who should be able to view some or all of your documents.

Purchase Document Uploads

Purchase up to 12 document uploads.

Upload Your Documents

Upload advance directives, power of attorney, will or any other documents that you want on hand in an emergency or legal situation.

You do not need to upload documents at the time of purchase.

Set Granular Permissions

For each document, choose who can have access.  You remain in full control!

Preparing Ahead of Time

Please prepare the following information/documents for your registration:

1) You will be asked to fill out your personal information, including your name, date of birth, ID number, phone number, email address, and home address.

2) You will also be asked to fill out information on the individuals you select (“designated individuals”) who will have access to some or all of your documents. You may select zero to five designated individuals. Information requested will include name, mobile number, email, optional landline number, optional home address, and optional relationship.

3) You may purchase up to 12 document uploads.  You do not need to upload documents at the time of purchase.  You can upload your documents at any time after purchase.

4) You may upload in PDF, JPG, PNG, and other image formats.  Suggested documents include List of Emergency Contacts, List of Allergies and Medications, Advance Healthcare Directive, Will, Trust, Power of Attorney, and Insurance Policy.

5) For each document you upload, you may choose whether a particular designated individual can have access, as well as hospitals and courts.

With the above information/documents gathered, you are now ready to register.
