RegistryAZ.com™ Inc. also stores contact information of your member authorized designated person(s), emergency contacts, and primary physician, along with other user-defined optional information.
Personal information collected by RegistryAZ.com™ Inc. upon registration, including name, address, state issued ID number, state issued DL number, passport number, phone number, email address, and date of birth, is collected for membership identification as well as for billing purposes.
RegistryAZ.com™ Inc. does not sell, loan, or rent member information or the information it stores on behalf of its members. Member uploaded and stored documents are released only to Member designated authorized persons who have satisfactorily proven their identification through legally valid credentials.
RegistryAZ.com™ Inc. strictly protects the security of the information Members provide. Personal information we collect is stored in a secured password-protected server with limited access. We carefully protect this information from misuse, unauthorized disclosure, alteration, or destruction.
The RegistryAZ.com™ Inc. website does not store any personally identifiable information about visitors to our site. RegistryAZ.com™ Inc. uses site visit information only in aggregate form to track the total number of visitors to our site. When RegistryAZ.com™ Members visit the Member Services portion of our website, the website uses a technology called “non-persistent cookies”. This information enables RegistryAZ.com™ Inc. to follow up with the requestor and/or verify your authorization to obtain documents.
Information RetrievalYour Advanced Health Care Directive, emergency contact information, Will, Durable Power of Attorney, HIPAA Release, Trust, Anatomical Gift Designation, and/or related personal information and any other member-provided information are available for retrieval via the RegistryAZ.com™ website. To receive these documents and information, the requestor must be able to provide the required identifying credentials. The documents and information will be supplied when the identifying information is provided and approved.
Upon enrollment, a member authorizes RegistryAZ.com™ Inc. to release their Advanced Health Care Directive, Will, Durable Power of Attorney, HIPAA Release, Trust, Anatomical Gift Designation, and/or related personal information to any authorized person or entity that has been identified by the member.
Updating Information and Account DataYou can update your personal and emergency contact information by visiting our website, www.RegistryAZ.com. On the website, your member information can be viewed and/or changed by logging in using your login credentials. When paying for membership services via our website, your credit card information is processed via a certified secure authority.
Members understand that RegistryAZ.com™ Inc. is not responsible for the validity or accuracy of any information stored by member on RegistryAZ.com™, including personal health information.
If a member has chosen to revoke or replace their Advanced Health Care Directive, Will, Durable Power of Attorney, HIPAA Release, Trust, Anatomical Gift Designation, and/or related personal information, it is the member’s responsibility to update their RegistryAZ.com™ member account. RegistryAZ.com™ Inc. shall not be liable for releasing information to authorized personnel or damages incurred as a result of member failing to update their stored information and documents. You may view and print your Advanced Health Care Directive, Will, Durable Power of Attorney, HIPAA Release, Trust, Anatomical Gift Designation, and/or related personal information from the RegistryAZ.com™ website; however, these documents cannot be edited or changed online. If you wish to add or make changes to your Advanced Health Care Directive, Will, Durable Power of Attorney, HIPAA Release, Trust, Anatomical Gift Designation, and/or related personal information stored with RegistryAZ.com™, you must upload the new document(s) to RegistryAZ.com™ using the member login portal.
CancellationYou may cancel membership in RegistryAZ.com™ at any time. To delete your account please use the “Delete Account” feature under the user’s Settings. Once you click it, you will see the permanent-deletion warning. You must enter your password to confirm your selection. Once your account has been deactivated, neither RegistryAZ.com™ Inc. nor you will be able to access any documents or contact information. Additionally, your designated authorized individuals will not have access to any previously accessible information. Any stored information will be permanently deleted from the RegistryAZ.com™ system. A refund of fifty percent (50%) will be given if you cancel your account within the first 30 days of creating it. There are no refunds for cancellations after thirty (30) days.
Disclosure Required by LawRegistryAZ.com™ Inc. may disclose personal information if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to the law or comply with legal process served on RegistryAZ.com™ Inc.; (b) protect and defend the rights or property of RegistryAZ.com™ Inc.; or (c) act in urgent circumstances to protect the personal safety of users of the RegistryAZ.com™ service, or the public.
Updates to Privacy PolicyWe may occasionally update this Privacy Policy. When we do, we will revise the “last updated” date at the top of the Privacy Policy.
